ENDOMETRIOSIS




Endometriosis is a condition when a tissue which usually grows inside the uterus, is found growing outside the uterus in abnormal places. for eg., ovaries etc.

Whenever the patient has menstrual cycles, this tissue also bleeds. This bleeding heals but form adhesions. So finally endometriosis results in restricted mobility of both sides of the fallopiantubes and there may be severe adhesions between ovaries, tubes, uterus and even intestines.

How can it present clinically?

During menstrual cycles, patients suffering from endometriosis may have severe pain. They may have pain during   intercourse and may also present with infertility (or) chronic lower abdominal pain.

How to manage an unmarried girl with endometriosis?

As said earlier, whenever the girl menstruates, she bleeds in ectopic endometrial tissues.  So   our medical management aims at reducing the number of cycles and to control the progress of the disease till she gets marrie.

Can all endometriosis be diagnosed using ultrasound?

No all endometriosis patients cannot be diagnosed by ultrasound unless it is an ovarian   endometriosis .Many a time; we diagnose endometriosis intra operatively, when a patient undergoes laparoscopy for treatment of infertility.

What is the advice given to previously diagnosed endometriosis patients who got newly married?

We advice them to plan for family early and not to postpone conception as the disease progresses during every cycle.         
                                                                        

What are the modalities of treatment available for management of endometriosis?

Both surgical and medical management.

Regarding medical management, we have GnRH analogue injections which we can use limitedly and tablets to be taken for a few months.

A female who has completed her family and having endometriosis, needs to be treated (or) not?

Even though the female has completed her family she needs to be treated if symptomatic like pain (or) any other symptoms due to adhesions.

What are the modalities of treatment for a female who has finished family and is having endometriosis?

POLYCYSTIC OVARIAN DISEASE


Polycystic Ovarian Syndrome is the most common cause of irregular menstrual cycles due to hormonal abnormalities. It is characterized by menstrual irregularity, increased androgen levels, obesity and Ultrasound Scan showing multiple small follicles in the ovary i.e. Polycystic Ovaries.

3 FACTORS AFFECTING FERTILITY: WEIGHT, AGE & EXERCISE

In the generations gone by no one might have ever thought of investigating the factors that influence fertility, this may be because their lifestyle hardly made it an issue. A sedentary lifestyle, unhealthy food habits and late marriages have made Weight, Age and Exercise an integral part of fertility issues.
The adverse effects of weight, age and lack of exercise are all reversible. It is up to women who are to understand the effects and take appropriate action. 
Here are a few pointers that can set you on the right track to fertility and good health.

Control Your Weight
Any woman planning to have a baby should be conscious of her weight and have a tight rein on it. This can be easily done by controlling the food you eat. Highly nutritious food that has less of fat and carbohydrates can work wonderfully for you. Overweight not only interferes with fertility it also brings complications in pregnancy. If you are overweight then it is best to reduce body weight before you conceive.

Similarly, underweight also influences fertility. Women suffering from anorexia tend to have their menstrual cycle disrupted which reduces the chances of becoming pregnant.

The ideal BMI for conception is between 20 – 25.

Consider Your Age
In women the peak fertility age is early 20’s and as they age and become 35 – 40 years their chances of getting pregnant reduces drastically. The same applies to men also, where their testosterone levels drop as they age and the volume and count of their sperms also drop. If the couple are in their early 20s chances of getting pregnant is high.

Effects of Exercise
It is now a well-known fact that exercise makes human life much more fruitful. The same is true of fertility. For those men and women who want to conceive it is advisable to exercise regularly. Care should be taken not to indulge in strenuous exercise especially by women because strenuous exercise affects ovulation cycles and thus the chance of getting pregnant decreases.

Normal exercise has not been observed to disrupt pregnancy, but exercise that strain you like high impact sports or scuba diving should be avoided especially during pregnancy. In men very strenuous exercise will definitely affect the sperm count.

ARE YOU SUITABLE FOR IUI?

The bane of infertility is being fast replaced with hundreds of options for fertility and childbirth. Infact Chennai is now recognized as a safe centre for laser assisted hatching, In-vitro Fertilization (IVF) and Intracytoplasmic sperm injection (ICSI) and Intrauterine Insemination (IUI). Other fertility options

The IUI may not be possible in all women and there are certain groups of women who cannot perform IUI like those suffering from endometriosis. The doctor conducts many tests including a HSG which is an X-ray for analyzing the health and structure of the fallopian tubes and to see whether they are blocked. If you are over 40 years or you have low ovarian reserve the doctor will recommend other fertility treatments like ICSI and IVF.

ICSI is the procedure of directly injecting the sperm into the oocyte; this treatment is also recommended if the male partner has low sperm count. The IVF is a procedure where a woman’s oocytes and a man’s sperms are harvested. The oocyte is then kept with the sperms and allowed to develop before implanting in the mother’s womb.
